Registration

We encourage parents to review our "Academics" link to the left as well as our "Preschool" link for parents interested in our Preschool.  All new families must register in person.  ALL incoming Kindergarteners must be registered in person, including the children coming from our own HMS Preschool.  Children do not have to be present, but at least one parent or guardian must attend and have the following paperwork:

 

In order to register for HMS, the following is required.  You must bring your child's:

·          Birth Certificate and child's Social Security Number

·          Baptismal Certificate (if your child was not baptized at Holy Martyrs)

·          Immunization Record (a copy of your pediatrician's records will suffice, but a signed form must be supplied before school begins in Sept.)

·          Registration Fee of $100 - This is a non-refundable fee and not applicable towards tuition.  Checks can be made 

         payable to Holy Martyrs School.

·          For children entering grades 1-8, please bring their most recent progress report or report card from the current school

 

Age Requirements

For children entering

Preschool:  must be three-years-old and fully potty-trained by September 1, 2010. 

Kindergarten:  must be five-years-old by September 1, 2010. 

Grade 1:  must be six-years-old by September 1, 2010. 

To register children in grades 2 through 8, please contact the school office for more detailed information.

Any parent that wishes to register a child, but can not attend Registration Night should call the school office to set up a time to come into school to register in-person.

 

Current HMS students re-register annually.  Re-registration forms are sent home to each family through school.  Completed forms and the re-registration fee must be submitted to the school office in order to be registered for the following school year.
